Have there been any real-life accidents or injuries on movie sets that show just how dangerous these action scenes can be?

One action scene that ruined many lives.

During the shooting of the film Bombay 405 Miles, during the climax sequence, a dangerous stunt was being performed by the stunt double of Shatrughn Sinha, named Mansoor. Mansoor was supposed to jump near a fire explosion. The action choreography was handled by M.B.Shetty (father of Rohit Shetty).

But there was some misunderstanding about the timing and place of jump as the coordinators changed the fire point at the last moment. The result was that Mansoor jumped right into the fire and was burned badly and d*ed on the spot. The whole shooting went to standstill and M.B.Shetty could never recover from this shock

M.B.Shetty was so aghast by this event that took to alcohol. He couldn't work and kept feeling guilty for the happening. His family fell into financial crisis and Ratna Shetty the second wife of M.B.Shetty (mother of Rohit Shetty) did odd junior artist roles to survive. Eventually M.B.Shetty also passed away within two years of this event.

But the curse of this accident didn't end here. The director of this film Brij Sadanah who had created small budget hits like Victoria no 203 and Chori Mera Kaam saw his career turning downhill from here. All his films from this film onwards failed badly and by the end of the decade he was in heavy debts. And on 21st October 1990 when his son Kamal Saransh was celebrating his birthday, Brij Sadanah fatally sh*t hos wife and his daughter and even fired at Kamal which injured one of his friends. And eventually he sh*t himself. All three of them d*ed.
